Information
Thank
you for choosing Women's Health Services. We appreciate your
allowing us to be part of your health care team. Please follow
these simple procedures to help our staff provide you the quality
time and care that we want you to have.
- Please inform us of any name, address or telephone number changes (both
work and home), as soon as it occurs.
- Please inform us of all changes in your insurance as soon as possible.
- Please unblock your phone if you are expecting a return call; a block
line could delay or prevent our reaching you.
- Please give us your name and date of birth when leaving messages, as
well as your doctors name. (Spell your name if it is easily misspelled and
speak slowly and clearly so we will be sure to have the correct
information.)
- Leave detailed messages on the pharmacy line, including name of drug,
dosages, and your pharmacy number. Please allow 24 to 48 hours for routine
refills., if at all possible.
Our office policy for test result notification is as follows:
- Pap smears- All normal pap smear results will be in the form of a
card that is mailed to you. Depending on the lab that is used, it can take
up to three weeks. All abnormal pap results will be given to you by a nurse
over the phone, once the doctor has seen the results and has made
recommendations.
- If you have not heard from us by phone or mail within 3 weeks please
give us a call.
- Lab (blood tests)- Most cultures are tested for 72 hours (3 days)
before a doctor can review them. We will call you as soon as the doctor has
reviewed your tests and made recommendations.
- If you have not heard from us in 7-10 days, please call us. We will
make every attempt to contact you sooner if medications need to be
changed or abnormal labs need immediate attention in the form of
prescription medication.

- Shots- While you do not need an appointment to receive your
injections, we can pull your chart and cut down on your waiting if you call
the day before and leave a message on the pharmacy voice mail. If you are
past due for an annual exam or follow-up appointment, you will need to
schedule it at that time. If you are on Depo-Provera it will be necessary
to get a prescription for the medication from our office, fill it at the
pharmacy, then bring the medication to our office so that we can administer
the injection. You will need to bring in the medication every 3 months for
as long as you are on Depo-Provera.
- Nurse and Doctor calls- In order for our physicians to spend
uninterrupted time with each of our patients during their office visits
call-in questions for the doctor must first go through a nurse. The nurses
are qualified to answer many of your questions or will check with the doctor
and return your call. It is necessary for us to prioritize our return calls
and non-urgent, simple or general information calls may not be returned the
same day. Calls regarding pain, bleeding, pregnancy complications, and
medication problems will be returned first. In order for us to properly
respond to your call please leave a detailed message with you correct name
and number. If an urgent call was not returned please let us know.
- After-hours calls- Our doctors share on call duties with other
doctors outside of our immediate group. These calls are also prioritized
according to their urgency. Please do not call after hours if your needs
can be met during regular business hours.
- By law, we can only give test results or medical history to the
patient, unless she is a minor child, or we have your written permission
to discuss your medical information with someone else. We have a form
available for you to sign if you wish to authorize this to include a
spouse, relative, or friend.

If you are a new patient and have access to a computer you may download, print, and complete the forms
below. This will be much easier, faster and accurate than completing
them in our office. Be sure then to bring them on your first appointment. You may
download each form
individually, or if you have a fast enough internet connection you may wish to
download the complete set of forms as one file.

| We are unable to complete disability paperwork or letters during your office
visit. These take time to fill out accurately and completely and for the doctor
to sign. We will be happy to do this paperwork, but please allow several days
for their completion. |
