Appointment Arrival Time: We recommend that you arrive at your appointment 15 minutes in advance. If you are running late, please call us so we can properly adjust our schedule.

Necessary information at check in: Be sure that you have your current insurance card, photo ID, and a list of your medications.

Expedient Check-in: If you give us your e-mail address when you book your appointment, we will e-mail you the PIN number to log in to the patient portal. Please go there to fill out your health history prior to your visit.

If our doctors are running behind schedule, we will make every attempt to notify you in advance. If the waiting time causes interference with your schedule, we will be glad to reschedule your appointment. In some instances, we are not able to notify you in advance of a scheduling delay. We appreciate your patience in these instances.
